Winter 2023 Internships

For students enrolling in AGC 339 for Winter 2023 please review and complete the following documents.  All documents must be submitted via email to aged@calpoly.edu.

Required Documents:

1. Internship Site Self-Assessment and Student Acknowledgment of Risks (signed by student, site supervisor, department on-campus internship coordinator) 
2. University Release of Liability
3. Complete CSU-Student Safety on Campus During COVID-19 (found through Learning Hub - SkillSoft in your portal)
4. Memorandum of Understanding (signed by student and site supervisor)
