For students enrolling in AGC 339, please review and complete the following documents.  All documents must be submitted via email to aged@calpoly.edu.

Required Documents:

1. Internship Site Self-Assessment and Student Acknowledgment of Risks (signed by student, site supervisor, department on-campus internship coordinator) 
2. University Release of Liability
3. Memorandum of Understanding (signed by student and site supervisor)
